Admission forms will be available from Monday to Friday, between 9:00 a.m. and 3:00 p.m., at PTV ICSE Section, Ground Floor, Subhash Road, Vile Parle East

Duly filled admission forms will be accepted from Monday to Friday between 9:00 am and 3:00 pm, at PTV ICSE Section, Ground Floor, Subhash Road, Vile Parle East (East). Contact: 90825 43535. The admission form fee of ₹500/- should be paid at the time of submission.
Documents List
Parents must submit the duly filled admission form along with the following documents:
1. Copy of Birth Certificate of the child.
2. Copy of Aadhaar Card of the child and both Parents.
3. Copy of Nursery / Jr. KG Report Card or ID card of Previous School of the child.
4. Caste Certificate (if applicable) of the child.
5. Copy of both Parents’ Educational Qualification Certificates (only the final highest degree).
6. Income Proof of both Parents – Copy of the latest Salary Slip / Form 16 / ITR.
7. Address Proof – A valid residential address proof in the name of either parent must be submitted at the time of admission (e.g., Aadhaar Card, Passport, Utility Bill, Rent Agreement).
8. Copy of Blood Group Report of the child.
9. Copy of Vaccination Report of the child.
10.Copy of Discharge Summary of the child.
